JAMA Aug20: Vitamin D No Use in Depression Prevention. Metformin Beneficial in Overweight Women on Fertility Treatment.

Many good articles as usual, most of them, of course, related to Covid infection: treatments, prevention, transmission, vaccines, etc. I do not review any of them unless it's got something to do to endocrinological issues, but highly recommend their reading.

Precision Nutrition, or What to Eat to Stay Healthy. "Precision" is a trendy word in medicine. In a blurry and unaccurate world like biology, precision has gained intrinsic value. Sometimes it's closely related to "personalized" medicine, i.e., the doctor will give you the exact medicine in precise amount with personalized posology, as if it were the result of your personal "Schrödinger's equation". According to the editorial, "Precision nutrition aims to understand the health effects of the complex interplay among genetics, microbiome, antibiotic and probiotic use, metabolism, food environment, and physical activity, as well as economic, social, and other behavioral characteristics. Only with a firm grasp of the contributions and interrelationships among these factors will it be possible to develop targeted nutrition guidance for diverse individuals in a highly diverse world" It presents a US Health Initiative called "The 2020-2030 Strategic Plan for NIH Nutrition Research" aimed at 4 main topics: 1) understand what we eat and the way it affects us; 2) Investigate what and when we should eat to promote health; 3) identify precise eating patterns across out lifespan; and 4) Use the food as a medicine for established diseases. In my opinion, if precision is already difficult in most fields of medicine, it reaches its highest rates in nutrition. You shouldn't forget that all living organisms have evolved mechanisms to adapt to environment, including availability of nutrients to maintain their vital functions. So, if medicine tries to adapt the nutrient intake to our needs what it's just doing is to go against the tide of what nature does. Can't come out well...

Vitamin D on depression. 18000 individuals >50 were enrolled in the VITAL trial. From their data the VITAL-DEP substudy found that daily vitamin D supplementation of 2000 UI of cholecalciferol was not better than placebo in preventing depression. I would have been surprised otherwise.

Althoug they explain in the DAPA-HF trial mortality reduction was similar regarding diabetes status, in Table 4 they recommend it for patients with type 2 diabetes and A1c >=7%.

Triglyceride Deposits in Heart Associated to Worse Outcomes in Diabetes. Triglyceride deposit cardiomyovasculopathy (TGCV) is a rare disorder encoded as an orphan disease. Diabetic patients have sometimes the same type of deposit. In this retrospective study, they divided 81 patients having a DES implanted in two groups, 7 with TG deposits and the rest without them as control group. One year later, they found that CG outcome was worse in the first group.

Metformin Beneficial for Ovarian Hyperstimulation Syndrome (OHSS) after IVF and probably for Pregnancy Rates, but only for overweight. In this metaanalysis Wu reviewed trials that randomized a total of 1123 women on fertility treatment to metformin or placebo. Metformin reduced OHSS (OR 0,43), statistical significant only for BMI >26. Pregnancy rates and live delivery was similar in both groups, but better for metformin also for those with BMI>26.
